Subject: Inquiry about the hiring process

Dear Mr.Taro Yamada,

I received the results of my interview from your HR department.
It is unfortunate that you decided that I am not the right person for your company, 
but I respect your decision and appreciate your taking the time to evaluate me.

I just wanted to ask you about the hiring process.
During the interview, I was asked to fill in a form about my family, 
personal relationships, and financial assets, among other things.

I was not sure if such information would be relevant to the hiring process.
Further, the form was not returned, even though I was told that it would be.

Your sincere response would be much appreciated.

また、審査するためにお時間を割いていただき感謝いたします。
貴社の採用プロセスについて少しお尋ねさせてください。

面接時に、とりわけ家族、個人的な人間関係、
金融資産等について記入するよう言われました。
そのような情報が採用プロセスに関係があるのだろうかと思いました。
また、その用紙は返却されると言われていたのですが、されませんでした。
